Confidentiality is a top priority at Apex Benefits Group, Inc. We are committed to maintaining the highest level of confidentiality with all of the information we receive from our clients. The privacy of individually identifiable personal information, including health information, (PHI), is a top priority and has always been handled with the strictest confidence and is only shared in order to provide you with insurance proposals, products and services. PHI includes both health information and other types of individually identifiable information, such as your employees' names, addresses, telephone numbers and social security numbers. The following notice describes our policies and practices with regard to collecting and disclosing PHI about our prospective, current, and former clients.
Confidentiality and Security of Your Personal Health Information
We maintain appropriate physical, electronic, and procedural safeguards to maintain the confidentiality and security of your PHI contained in our records. We restrict access to PHI about you contained in our records to our employees who need to know that information to provide products or services to you. You can be confident that we will not sell your PHI, and that we will not share it with anyone for purposes unrelated to our business functions, except as required by law.
Categories of Information That We Collect
Health information that we collect about your plan and its members falls into the following categories:
- Information that we receive from you on applications and other forms;
- Information that we receive about your transactions with our affiliates, others or us;
- Information that we receive from other entities that perform a business, professional, or insurance function either for you or for us on your behalf; and
- Information that we receive from your plan members or policyholders, or their medical professionals, for claims-related services.
Parties From Whom We Collect Information
In providing services to our clients, we collect information from many sources, as described above. Collection of this information is authorized by the applicable regulations, including HIPAA's Privacy Rule, as long as we receive it on your behalf for purposes of treatment of your plan members or policyholders, for payment of claims, or for your plan's claim-related business operations. We commonly receive this information from you. However, you are also authorized by the regulations to allow us to obtain PHI directly from your plan members or policyholders, if such is necessary to assist in claim payment or your plan's business operations.
Categories of Parties to Whom We May Disclose Information
We may disclose PHI about you to companies that provide services to us, and those that we communicate with on your behalf, such as reinsurance carriers, third party administrators, or other claims-related entities. This information is shared in order to assist you with matter related to claim payment and your claim-related business operations, and just as described above, sharing it for that purpose is expressly authorized by the applicable regulations. We may also disclose PHI to other third parties, but only as permitted or required by law, or as authorized by you.
Accuracy of Your Information That We Possess
We strive to maintain the accuracy of your information in our possession. In order to help us maintain accuracy, you have the right to reasonably access your information. If you believe any of your information in our possession is inaccurate, you may request that we amend, correct or delete information you believe to be erroneous. If we concur with your conclusion, we will amend, correct or delete the information in question.
Confidentiality of Electronic Communication
Should you send us your questions, comments or information via email to any mailbox provided by our company, we will share your correspondence with our employees or agents of our company most capable of addressing your questions and concerns. We will retain your communications until we have done our very best to provide you with a complete and satisfactory response. Ultimately, we will either discard your communication, or in some cases keep it in your secure file for future reference.
